Dear Austin Perio Health Patients and Patient Families, Appointment Confirmation: We kindly ask that you reschedule your appointment if you or anyone in your household has one of the following symptoms or conditions: -Fever of 100.4 oF or higher We request that you wait to reschedule until you have been symptom-free for a period of 14 days. Patient Check-in: If you feel more comfortable waiting in your vehicle until your appointment time, just call the office Austin Office Phone Number 512-454-6861 when you arrive, and we will contact you as soon as we are ready to take you to the treatment room. For new patients, please have patient forms filled out prior to your appointment and emailed to: [email protected]. We are happy to email your patient registration forms to you. Please provide us with your email address when scheduling your appointment. BEFORE Entering the Office: Please use the sink in our building hall lobby bathroom to wash your hands with soap and water immediately prior to entering the office. We ask that you please wear your face mask to your appointment, and at all times in the waiting room. Respectfully, we will refrain from polite handshaking until further notice. AFTER Entering the Office: We request that patients use hand sanitizer after entering the office. All patients who enter the office will have their temperatures taken with an infrared touchless thermometer. If you have had fever, chills, fatigue or any symptoms of COVID-19, or have been in contact with anyone who has COVID-19, we will need to reschedule your appointment. If you arrive at the office with a fever above 100.4F, we request that you see your physician and we will reschedule your appointment. Please review the COVID-19 Patient Disclosure Form prior to your visit to screen yourself for symptoms. We will distribute this form at each appointment to be filled out immediately following check-in. Extra Family, Parents, & Drivers: To protect all patients, please come to your appointment alone, unless you are being sedated and require a driver. If your appointment requires a driver, or for our younger patients who need a parent/guardian, we allow only 1 guest per patient. If anyone else accompanies you to your appointment, we request that they wait in the car. Additional steps we take to protect the health of our patients & staff: · We always have and will continue to adhere to CDC and OSHA infection control guidelines. · Clinical staff will wear N-95 Respirators with face shields. · Hourly decontamination of all frequently touched non-clinical surfaces.
